This review will discuss the basic metabolic panel (BMP) and comprehensive metabolic panel (CMPT) normal ranges, causes of abnormal results, and your role as a nurse. A metabolic panel blood test assesses: electrolytes, renal function, and liver function. Note: The BMP and CMP both assess the electrolytes and renal function, BUT the CMP also checks the liver function. You can buy it online at DiscountedLabs The comprehensive metabolic CMP panel or chemical screening, (CMP) is a panel of 14 blood tests which serves as an initial medical screening tool to review overall health. The CMP functions as a check for kidney function, liver function, and electrolyte and fluid balance. These are the tests included: 1. Sodium (Na) 2. Potassium (K) 3. Chloride (C) 4. Carbon Dioxide (CO2) 5. Albumin (Alb) 6. Alkaline Phosphatase (ALP) 7. Bilirubin (Bil) Total 8. Aspartate Transaminase (AST) 9. Alanine Transaminase (ALT) 10. Blood Urea Nitrogen (BUN) 11. Total Protein 12. Calcium (Ca) 13. Creatinine (Cr) 14. Glucose Liver Function Tests Liver function tests, also known as the hepatic panel, are laboratory tests that help measure how well the liver is working. The liver carries out many vital bodily functions; when it is not working properly, levels of various enzymes, proteins, and other substances in the blood may rise or fall. Elevated liver enzyme levels may be a sign of liver damage caused by factors such as viral hepatitis, heavy alcohol consumption, or drug toxicity. Alanine transaminase (ALT): formerly called SGPT, ALT is an enzyme normally present in liver cells. When these cells are damaged, ALT is released into the bloodstream. A normal ALT level for adult men is 0-50 International Units per liter (IU/L); levels are somewhat lower in women. While mild elevations are common (specially in people who exercise), an ALT level more than 2.5 times the upper limit of normal (ULN) is cause for concern. In general, upward or downward trends in ALT are more informative than a single measurement. Aspartate transaminase (AST): formerly called SGOT, AST is another liver enzyme that may spill into the blood when liver cells are damaged. AST levels may also be elevated in people with muscle damage. A normal AST level for adult men is 0-45 IU/L. Bilirubin: bilirubin is a pigment released when red blood cells are broken down. A normal bilirubin level is 0.1-1.5 mg/dL. An elevated level (hyperbilirubinemia) may indicate liver damage, impaired bile flow, or excessive red blood cell destruction. High bilirubin levels can lead to jaundice (yellowing of the skin and whites of the eyes). Alkaline phosphatase (AP): elevated AP levels may signal obstructed bile flow or bone destruction. A normal level is 35-115 IU/L. Kidney Function Tests Blood urea nitrogen (BUN): nitrogen is a metabolic waste product that is normally filtered out by the kidneys and excreted in the urine. A normal BUN is 8-20 mg/dL. Elevations may indicate kidney dysfunction, high protein intake or a body fluid imbalance (e.g., dehydration). Creatinine: this waste product of protein metabolism is also normally excreted by the kidneys. A normal blood creatinine level is 0.6-1.5 mg/dL. Elevated creatinine levels may indicate kidney damage. Creatinine is used in calculating creatinine clearance (creatinine clearance below 60 may indicate impaired kidney function). It is one of the most commonly ordered lab tests. The CMP gives your healthcare practitioner important information about the current status of your body's metabolism (hence the name metabolic panel). The CMP provides information on your blood sugar (glucose) levels, the balance of electrolytes and fluid as well as the health of your kidneys and liver. Abnormal results, and especially combinations of abnormal results, can indicate a problem that needs to be addressed and may require additional testing. The CMP includes the following tests: Glucose - the primary energy source for the body's cells; a steady supply must be available for use, and a relatively stable level of glucose must be maintained in the blood. Calcium - one of the most important minerals in the body; it is essential for the proper functioning of muscles, nerves, and the heart and is required in blood clotting and in the formation of bones. Proteins Albumin - a small protein made by the liver; it makes up about 60% of the total protein in the blood. Total Protein - measures albumin as well as all other proteins in blood; proteins are important building blocks of all cells and tissues and are essential for body growth, development, and health. Electrolytes—these are minerals found in body tissues and blood in the form of dissolved salts. Electrolytes help move nutrients into the body's cells and help remove wastes out of the cells. They help maintain a healthy water balance and help stabilize the body's acid-base (pH) level. The following 4 tests are commonly called electrolytes: Sodium - vital to normal body function, including nerve and muscle function Potassium - vital to cell metabolism and muscle function, helping to transmit messages between nerves and muscles Bicarbonate (Total CO2) - helps to maintain the body's acid-base balance (pH) Chloride - helps to regulate the amount of fluid in the body and maintain the acid-base balance Kidney Tests Blood urea nitrogen (BUN) - waste product filtered out of the blood by the kidneys; as kidney function decreases, the BUN level rises. Creatinine - waste product produced in the muscles; it is filtered out of the blood by the kidneys so blood levels are a good indication of how well the kidneys are working. Liver Tests Alkaline phosphatase (ALP) - enzyme found in bone, the liver, and other tissues; elevated levels of ALP in the blood are most commonly caused by liver disease or bone disorders. Alanine amino transferase (ALT, SGPT) - enzyme found mostly in the cells of the liver and kidney; a useful test for detecting liver damage Aspartate amino transferase (AST, SGOT) - enzyme found especially in cells in the heart and liver; also a useful test for detecting liver damage Bilirubin - an orange-yellow pigment, a waste product primarily produced by the normal breakdown of heme; heme is a component of hemoglobin, which is found in red blood cells (RBCs). Bilirubin is ultimately processed by the liver so that it can be removed from the body.

This is another very common test in the ICU for our critically ill patients and gives us a lot of useful information to try and help diagnose and treat our patients. Understand their purpose, normal and abnormal values, and what would lead to high or low levels of specific tests is imperative in the ICU. Complete blood count (CBC) test result interpretation explained for nursing exams and NCLEX. A complete blood count (CBC) is a blood test used to assess a patient's general healthiness. It can help determine infection, anemia, certain types of blood cancers, etc. A CBC with differential will assess the 5 types of white blood cells (basophils, eosinophils, monocytes, lymphocytes, neutrophils). The procedure for a CBC collection consists of removing blood from a patient's vein via a needle. The blood is collected into a lab tube. The tube of blood is sent to the lab, and the laboratory will take the blood specimen and run a complete blood count test. The results will be sent to the nurse and doctor. The complete blood count checks the white blood cell count (WBC), red blood cell count (RBC), hemoglobin/hematocrit, red blood cell indices, and platelets. DKA LAB VALUES Are you struggling with learning lab values in nursing school? In this video, I'm going to walk you through the 7 main lab values you need to assess for in a patient with diabetic ketoacidosis (or DKA for short). During your nursing assessment, here are 7 lab values you'll need to assess in a patient with DKA: Snag your FREE lab values cheat sheet right here: 🤍 1. Blood glucose level. During diabetic ketoacidosis, the body cells can't use glucose for energy. This causes a build up of glucose in the blood, which leads to hyperglycemia (increased blood glucose). 2. Urine Ketones: During DKA, the body cells are using fat for energy instead of glucose. And when fat is broken down, ketones are released. These ketones will show up in the urine in a patient with diabetic ketoacidosis because there will be ketones circulating in the body. 3. Urine Glucose: Similar to the hyperglycemia and blood glucose level we listed above, because there is so much glucose in the blood (the body cells can't use it for energy), the kidneys will try to come to the rescue and attempt to get rid of it through the urine. 4. Potassium (this is a KEY POINT!): Potassium usually hangs out inside of cells. But during hyperglycemia, it moves outside of the cells. This causes the potassium lab value to increase during diabetic ketoacidosis. When your patient is treated for DKA (especially with insulin), the potassium level will decrease. You MUST be aware of this in nursing school when you're treating a patient with DKA. Always assess their potassium lab value throughout treatment to make sure their potassium level stays within normal range. 5. Arterial Blood Gas Analysis (ABGs - pH, the HCO3, and the PaCO2 lab values): As we said before, when the body can't use glucose for energy, it uses fat instead. And when fat is broken down, ketones are produced. Ketones are acids, and the more acids there are in the blood, the lower the pH value will be, and the more the ABG values will be off. In this video Doctor O'Donovan explains the CRP blood (lab) test including 1. What CRP is 2. Why the test is used 3. What the result of the test might mean. What is being tested? C-reactive protein (CRP) is an acute phase reactant, a protein made by the liver that is released into the blood within a few hours after tissue injury, the start of an infection or other inflammation. Increased concentrations in the blood can be found after a heart attack, in sepsis, and after a surgical procedure. It is often the first evidence of inflammation or an infection in the body, with rising concentrations frequently preceding pain, fever or other clinical indicators. The concentration of CRP in the blood can jump a thousand-fold in response to inflammation and can be valuable in monitoring disease activity. The CRP blood test is not diagnostic but it provides information to the doctor as to whether inflammation is present. This information can be used by the doctor in conjunction with other factors, such as signs and symptoms, physical examination and other tests to determine if someone has an acute inflammatory condition, or if they are experiencing a flare-up of a chronic inflammatory disease. The doctor may then follow up with further testing and treatment. How is the sample collected for testing? A blood sample is obtained by inserting a needle into a vein in your arm. Is any test preparation needed to ensure the quality of the sample? No test preparation is needed.
